Customized Community Supports increase independence by establishing or strengthening interpersonal relationships & networks. 

Alta Mira Specialized Family Services Inc.’s Customized Community Supports (CCS) provide individuals preferences and choices designed to measure progress toward desired outcomes specific to an Individual Service Plan (ISP). Supports are designed to increase independence and establish or strengthen interpersonal relationships by joining networks and participating in typical community life.

CCS activities include adaptive skill development, adult educational supports, citizenship skills, communication and social skills, self-advocacy, informed choice, community integration, and relationship-building instruction.

Services are provided in Bernalillo, Sandoval, Torrence, and Valencia counties.

Questions about outcomes?

Outcomes from CCS may include an enhanced capacity for self-determination development of social networks that allow individuals to experience valued social roles while contributing to their community by establishing lasting community connections.
